You'll also receive a personalized care plan based on this evaluation. At this point, you can choose to move forward with just the initial consultation or opt into our annual care package for ongoing support.

What services are included in the annual care package?

The annual care package, priced at $10,200, includes unlimited house calls, telehealth consultations, and direct access to our network of trusted concierge partners. You will also receive on going care coordination, continuous updates to the care plan, and priority access to care. This package is designed to provide comprehensive, worry free support for your loved one throughout the year.

Our collaborative partner services are billed separately from the care package. This ensures do you have the flexibility to choose only the services that best meet your loved ones needs

What if I opt into the annual plan and my loved one decompensates, passes away, or enters hospice/nursing home care?

We understand the circumstances can change, and the health of your love one may evolve over time. If your loved one decompensates, passes away, or enters hospice or a nursing home, we will work with you to prorate the remainder of the annual plan, ensuring you only pay for the services received. Any difference will be refunded with a net 60 - day return policy. Additionally, as part of our commitment to supporting caregivers, we offer the option to give the remaining care package to another family member or a fellow caregiver who could benefit from our service. This flexibility ensures that no part of the plan goes to waste and continues to provide value to those who need it most.
